In medicine you go from (and excuse my gendering for clarity) Ms/Mr to Dr. to Ms/Mr when you become a high-up consultant.
In media, you go from low billing on credits, to top billed, to the end of credits after “with” or “and”.

Ms/Mr is for surgeons. This is a hangover from when barbers used to do minor surgeries and used to be separate from doctors. Now surgeons take pride in being distinct from doctors.
High up consultants remain Dr if they are medical doctors.
